| 1pm start – Technical poomsae as per nomination form. All Pairs, Team & Creative poomsae will begin at the completion of technical poomsae. | |

| This is the receive date by organisers. You must have your completed form with payment to your instructor by the Tuesday 15th July 2008 to allow necessary time for postage. | |
| Notes: | All players and their clubs are to supply their own protective head, shin, chest, groin, arm & mouth guards. All players to have nails cropped and no jewellery or attachments are allowed during competition. Mouth guards are compulsory. All late, incomplete or incorrectly filled out forms will not be accepted. Please note: No personal cheques will be accepted. Money orders and Head instructor’s cheques only. |
